Evidence shows that seeing the same GP and healthcare team regularly leads to better health outcomes. By registering with MyMedicare, general practices and patients are mutually agreeing that they have an ongoing care relationship, an important foundation for continuity of care and more integrated, person-centred care.   MyMedicare, alongside other changes aims to strengthen Medicare for all Australians, and provide additional benefits for patients, and benefits for general practices and healthcare providers.

MyMedicare is voluntary and creates a unique relationship between a patient and one general practice: designating them as their preferred general practice for ongoing care. General practices can register through PRODA, and patients can register quickly and easily through Medicare Online or the Express Plus Medicare Mobile app, These digital systems also provide access to a range of other useful information for patients such as Medicare card details, make or track Medicare claims, add new family members (babies), safety net information and organ donation information. Patients can also register by completing a paper registration form and returning this to their general practice.

Choosing a MyMedicare Practice

When choosing a MyMedicare practice patients should register with the general practice that:

  • Provides care related to a chronic condition (a condition that will last >6 months, for example, ongoing pain, asthma, diabetes, arthritis, kidney disease, osteoporosis, COPD, hypertension, heart disease).
  • Is their usual general practice where they currently (or plan to) receive ongoing care and support.
  • Regularly provides longer (level C and D) telehealth bulk billed appointments.
  • Is delivering the General Practice in Aged Care Incentive (regular visits and care planning) for permanent residential aged care home residents.

Future MyMedicare Changes

MyMedicare will continue to evolve to promote continuity of care for primary care providers and patients to strengthen Medicare and improve health outcomes.

The following Medicare items and incentives will have links with MyMedicare from 1 November 2025:

  • The new Bulk Billing Practice Incentive Program – requires practices and general practitioners to register with MyMedicare to participate.
  • GP Mental Health Treatment Plans – changes to the Better Access program encourage patients to access GP Mental Health Treatment Plans through their MyMedicare registered practice, or their usual GP.

Additionally, the Review of General Practice Incentives includes recommendations to link future practice incentives with MyMedicare.

National PHN MyMedicare Implementation Program

The implementation of MyMedicare requires targeted activities for priority populations, changes to models of care, business practices, consumer engagement, and development of new systems, tools and training for both primary care workforce and PHN teams

PHNs, together with the Department of Health and Aged Care (DoHAC) and key stakeholders are working together to support local general practices and health providers to implement changes to prepare for and participate in MyMedicare.  

Change management is a key part of ensuring that the change is successful in the long-term and involves applying a structured process and a set of tools for supporting people through change. The program change management approach is outlined in the below diagram, and incorporates a range of coordinated activities specific to the populations engaged through each phase of the MyMedicare rollout. 

The PHN National MyMedicare Project supports implementation of MyMedicare, using change management methodologies across seven workstreams:  

  • Governance,  
  • Communication,  
  • Change Management,  
  • Quality Improvement and Safety,  
  • Upskilling of PHN staff,  
  • Data and Data Sharing, and  
  • Involvement in Co-design. 

 

 

An iterative change management approach, including review points, to deliver the change management plan, including the stakeholder engagement and communications plan, and evaluation plan aims to ensure that PHNs and health care stakeholders are supported throughout the change program, whilst ensuring that the Implementation project can: 

  1. Develop and implement a coordinated approach that can be applied regionally by each PHN. 
  2. Develop key tools to enable practice support teams in their engagement with general practice. 
  3. Ensure this strategy is informed by data and evidence to enable and enhance implementation. 
  4. Maximise the PHN capability (coordination, facilitation, change management and capacity building) to support successful implementation. 

PHN National MyMedicare Program Governance and Collaboration Model

The National PHN MyMedicare Implementation Program is a partnership between DoHAC and Primary Health Networks. This dedicated resource:

  • Supports and coordinates with all PHNs  
  • Efficiently develops consistent resources and communications for use by PHNs locally  
  • Dissemination of innovation by sharing PHN exemplar work across PHN Networks 
  • Collates PHN local implementation FAQs, issues and opportunities to inform DoHAC and reform improvement opportunities 
  • Advises DoHAC on reform implementation and change management 
  • Evaluates national program delivery 

 

Each of the 31 local PHNs are represented as a petal in the model is:

  • Collaborating with the national program to enhance efficiency
  • Locally engaging with stakeholders in change management activities to support the local MyMedicare implementation, and
  • Sharing experiences and locally developed solutions, resources/tools and case studies to scale up success
  • Contributing feedback from stakeholders to inform collaborative activities, issues and improvement opportunities

PHN MyMedicare Implementation Program Resources

Each Primary Health Network localises resources published by the national Program and provides hands on support to general practices and other stakeholders in their region to support change management.
